Elements of Competency Performance Criteria              
Element: Provide instruction and information to clients.
  • Adhere to scope of own role in providing medical information according to regulatory requirements.
  • Read information and check its currency and accuracy prior to provision of information to clients.
  • Instruct and inform clients as per health professional request, ensuring consistency of message.
  • Recall clients as per practice protocols and health professional instructions.
  • Interpret and manage the medical practice reminder system according to procedures.
  • Monitor ‘did not attend’ clients and follow up according to organisational policies and procedures.
  • Identify critical client appointments and follow up according to protocols if clients fails to attend.
  • Accurately record communication with, and in regard to, clients.

  • Facilitate information exchange programs designed to encourage and support client participation.
  • Pro-actively identify and follow up missing information.

  • Make service provider and health care program information readily available to colleagues.
  • Liaise with other service providers and organise required care as per health professional request.
  • Coordinate case discussions amongst providers, when required to support client care.
  • Complete required documentation for referral as per health professional request.
  • Provide complete and correct information to clients.

Assessment Tasks

Copy and paste from the following data to produce each assessment task. Write these in plain English and spell out how, when and where the task is to be carried out, under what conditions, and what resources are needed. Include guidelines about how well the candidate has to perform a task for it to be judged satisfactory.
Copy and paste from the following performance criteria to create an observation checklist for each task. When you have finished writing your assessment tool every one of these must have been addressed, preferably several times in a variety of contexts. To ensure this occurs download the assessment matrix for the unit; enter each assessment task as a column header and place check marks against each performance criteria that task addresses.
